Vaccine Errors: Prevention, management and open disclosure

This course has been designed to provide an overview of how to prevent errors of vaccine administration, the management of a vaccine error and to demonstrate the open disclosure process. At the completion of this course the immunisation provider will be able to:

• Identify the processes that reduce the risk of vaccine-related errors occurring
• Identify the necessary steps and management if a vaccine error occurs
• Recognise the importance of an open disclosure discussion and providing support to all individuals involved
• Identify the underpinning concepts of open disclosure and the important steps involved in the process
